Making A Claim

Dealing with emotional situations - whether it's a disability, death or job loss - is difficult. We will work with you to make the claims process as easy as possible.

    Following are the 5 easy steps you need to complete to make a claim:
    1.   Click here to download the claim form or if you would prefer to receive one by post,
    call  061 73 73 73* and we'll be happy to send you one.
    2. Fill in the interactive claim form completely

    Read each field carefully, click in the appropriate fields and begin typing

    Please use ALL CAPS

    You can move from one field to the next by pressing the Tab button. Shift+Tab will move your cursor backwards

    3. Print the claim form.
    4. Sign and date the form at the bottom.
    5. Post your claim form with required evidence and certification to the address shown at the top of the claim form.**

What happens after completing the form?

Once we have received your claim form, we´ll notify you by post of our decision. If your claim is accepted, you´ll receive payment promptly.
